ACCIDENT CHECKLIST
Fill out as soon after the accident as possible.
	If anyone is injured, call 911 -- seek medical help immediately.
	Write down the license plate number or numbers of all vehicles involved. If you exchange license or insurance information with other drivers, make sure it is complete.
	Get the names and contact numbers of any who witnessed the accident.
	If there is an opportunity while waiting for assistance, write down all recollections of what you saw, heard and experienced. Important facts could be lost if not recorded as soon as possible after the accident.
