MOTOR VEHICLE ACCIDENTS

ACCIDENT CHECKLIST

Fill out as soon after the accident as possible.
checkmark If anyone is injured, call 911 -- seek medical help immediately.
checkmark Write down the license plate number or numbers of all vehicles involved. If you exchange license or insurance information with other drivers, make sure it is complete.
checkmark Get the names and contact numbers of any who witnessed the accident.
checkmark If there is an opportunity while waiting for assistance, write down all recollections of what you saw, heard and experienced. Important facts could be lost if not recorded as soon as possible after the accident.
